Introduction: The purpose of all kinds of blood manipulation is the increase of haemoglobin mass (Hb-mass) and thereby the improvement of blood oxygen transport. A regular control of Hb-mass is therefore intensively discussed in the anti-doping fight. The aim of this study was to check the feasibility of Hb-mass measurements under the conditions of Ironman competitions. Methods: Hb-mass of a male elite triathlete (33 yrs) was determined using the optimized CO-rebreathing technique before and immediately after two Ironman competitions which were performed within one year at the identical place. Results: The competition time was in the first year slightly above and in the second year slightly below 8.00h, respectively. Mean Hb-mass (ñ standard deviation) of all measurements was 14.1ñ0.3 g/kg which was lower than the mean value (ñSD) of professional triathletes (15.0ñ0.8 g/kg) also participating in Ironman competitions. Hb-mass did not differ between the two years (977 g vs. 980g) and did not change during the Ironman competitions (differences of 18g and 9g, respectively).
Conclusions: 1. The Hb-mass of the examined elite athlete was very constant over a long time period and was not influenced by long-lasting high intensity competitions. A blood manipulation was therefore unlikely in this athlete.
2. The relatively low Hb-mass observed in this athlete was not a hindrance for world class performances.
3. The application of the CO-rebreathing method two days before the competition did not seem to exert negative effects on performance in this athlete.
